So the grappling hook's cable threads through the carabiner-type clip on the shoulder of the body armor, then connects to that little reel on the back of the belt. Activating the reel mechanism reels in the cable and pulls the agent upward toward where the grappling hook is anchored, and the carabiner on the shoulder provides a point of stabilization that keeps the torso parallel to the cable so that the agent isn't being hoisted up butt-first.
